2011 OPEN ENROLLMENT ANNOUNCEMENT

Employees of Bay Mills Indian Community can make changes to your Insurance Coverage during the open enrollment period:  DECEMBER 1st THROUGH 30th (4:30 p.m.), 2011.   Employees can ADD or DROP Dental & Vision coverage at this time, as well as ADD or DROP a dependent or spouse (Spouse\Dependent changes are also accepted within 30 days of the event: marriage, birth, etc.)  Contact Stacey Walden at Human Resources, (906) 248-8522 for more information or to make changes. 

WATCH FOR THE OPEN ENROLLMENT NOTICE INCLUDED WITH YOUR PAYCHECK\STUB ON 11-24-10.

NO MAJOR CHANGES IN COVERAGE LEVELS:
    Deductible ($2500 single\$5000 family) co-insurance (20% in-network), and co-pay maximums ($2500\$5000) will NOT change for Plan Year 2011.  However, the following changes will be implemented in response to health care reform (Patient Protection and Affordability of Care Act-PPACA):  Removal of Lifetime Maximums on Health Care, Removal of Yearly Maximum on Preventive Care, Expansion of services that qualify as 100% paid Preventive Care, Dependent age increase to 26 years of age, and the Removal of the Family Continuation Rider as dependent status no longer limited by financial dependency, marital status, or school enrollment.

INCREASE IN EMPLOYEE PREMIUM SHARES:  Employee Premium Shares that are deducted out of every bi-weekly pay check will increase to reflect new premium rates established by BC\BS of Michigan.  For specific information regarding YOUR premium share amount, refer to Open Enrollment Notice (included in your 11-24-10 pay check) or contact Stacey Walden at 248-8522.
 
INCREASE IN DENTAL & VISION PREMIUMS:  There are no plan changes to the D\V Coverage for 2011.  However, employees enrolled in D\V coverage will have their payroll deducted premiums adjusted to reflect the increase in premium rates established by BC\BS of Michigan.   For specific information regarding YOUR premium share amount, refer to your Open Enrollment Notice (included with 11-24-10 pay check) or contact Stacey Walden at 248-8522. 
Employees wanting to ADD or DROP DENTAL & VISION must submit a new BC\BS Change of Status Form during Open Enrollment.

INFORMATIONAL SESSIONS:  Dec. 2nd at 10 a.m., Dec. 9th at 3 p.m., December 16th at 10 a.m. at the Ellen Marshall Human Resources Department OR anytime by appointment (call Stacey at 248-8522). 

REMEMBER:  Employee health coverage is voluntary and there is no obligation to maintain health coverage through BMIC.  Employees wanting to cancel their coverage must complete a BCBSM CHANGE OF STATUS & WAIVER FORM at the Human Resources Department.  Those cancelling will not have an opportunity to enroll until December of 2011.
